  • In order to accurately determine the subject matter and difficulty level of the course to be taken at another institution, the evaluator requires documentation. A CTR-5 form must be submitted to the Office of Credit Evaluation in Schleman Hall prior to evaluation. Students seeking approval for Study Abroad courses should bring their preliminary course approval form to the meeting.
